Where Do I Get the Forms to Request a Hearing

The best place to start for all forms and paperwork in order to request a hearing before an administrative law judge is www.ssa.gov. This site contains all the forms needed for requesting a hearing, along with all instructions on how to file.  On the main page click on Forms and Publications then Social Security Forms. Under the Other Forms section, you will find the forms and instructions for filing for a hearing.

You will need all of these forms in order to request a hearing

  1. HA-501-US Request for Hearing by Administrative Law Judge
  2. SSA-3441 Disability Report Appeal
  3. SSA-827 Authorization to Disclose Information to SSA
  4. HA-4631 Claimants Recent Medical Treatment
  5. HA-4632 Claimants Medications
  6. HA-4633 Claimants Work Background (this is needed if you have worked since you have filed for any disability benefits)
  7. SSA-1696 Appointment of Representative (this is needed if you are appointing a representative to your case, it will need to be signed by your representative prior to submission)
